I like the Coaching Success Assessment form, but I hesitate to ask a busy professional to fill it out. The form would be more useful if it were shorter. Perhaps 1/2 the size?

I would think that you would only ask someone to do this periodically. But the most useful part of this assessment to me is to assess how I’m doing as a coach. It serves as a reminder of some things that I may not have been attending to in my coaching sessions.
